PLAYER REGISTRATION - YEAR 2012

Introduction / Conditions

Welcome to the on-line registration and payment gateway for Kew Comets Junior Football Club. Please ensure you review and understand the terms and conditions which appear as part of this Registration form. This site includes a secure on-line payment gateway.

Registration costs for 2012 will be $215 for a single player, and $390 for a family (two or more players). If you register by 31st January 2012 you will be eligible for entry into our prize draw.

The Club must advise the YJFL how many teams we intend to field in early February. To avoid the disappointment of missing out, please make sure you register as early as possible.

We encourage you to register and pay online by credit card, and if you choose this method you will need your credit card details (Mastercard/Visa) at hand to complete this registration.

Players must be paid up by season commencement, to play with the Comets.

4.1 Players and Parents Consent & Declaration

Privacy
I agree that the information provided herein may be used for distribution of club related information.

Medical Consent
In the event of illness or accident to my child at a match or practice and where the Kew Junior Football Club person in charge is unable to contact me or it is otherwise impractical to contact me, I authorise them to: (a) Administer any medical attention as may be deemed necessary by a medical practitioner; (b) Administer first aid that the person in charge may judge to be reasonable and necessary; and (c) Arrange for transport to a nearby medical clinic or hospital as necessary.

Club Membership
Properly financial and registered players are "Junior Members" of the Club in accordance with the Club Rules. Parents/Guardians of Junior Members become "Voting Members" of the Club (max two per family) in accordance with the Club Rules by completing the details below on this registration form. Parents/Guardians must complete this form in order to be eligible to vote at General Meetings.

Photography
Photographs of players may be taken from time to time and used for club promotion or by the YJFL.

Team Selection Policy
The Club, through the Football Sub-Committee is solely responsible for team selection. The team selection policy can be found on the Kew Comets website at http://www.kewcomets.org.au.
